Why do the smoothies I make always turn out wrong?

I follow the directions on every smoothie thing I get (ice, cream, liquor, frozen fruit, whatever it might include),
and it always turns out as slush instead of a smoothie!
Why is this so?
Is it because of my blender?
Theres always chunks of ice and all the contents float to the top while the liquids seperate on the bottom.
I noticed how it’s done at bars and restaurants, but they turn out perfect.
What can I do to fix this?

    I believe its because of your blender.
    That same thing always used to happen to me
    but then i got a new blender and it was great
    there were no ice chunks or anything

    My rule of thumb is I never ever use ice, I think it just makes for a chunky watery mess of a smoothie but most people disagree with me. A lot of it most likely does have to do with the blender however, most models really fall short of what they should do. Also keep in mind that liquids of different densities will begin to separate.
